The La Côte Foundation for Home Care and Prevention manages the medical-social centers in the districts of Nyon and Morges, as well as Espace Prévention in Nyon and Morges and CMS+ in Etoy.

The medical-social centers offer home care and assistance services. Care is provided 24 hours a day by a multidisciplinary team (nurses, occupational therapists, social workers, multi-skilled assistants, family helpers) in collaboration with other partners in the network: hospitals, CTR (treatment and rehabilitation centers), EMS (medical-social establishments), etc.

Services:

• nursing care;
• health, support, and supervision visits;
• end-of-life support;
• rehabilitation treatments for daily living;
• neuromuscular and orthopedic recovery;
• recommendations and selection of assistive devices;
• assistance and advice with administrative and financial procedures
• home help with washing, cleaning, laundry, or ironing;
• assistance for the family;
• home-delivered meals;
• watching over and keeping company;
• biotelemonitoring devices (Secutel);
• guidance and organization of placements, short stays, and temporary accommodation (UAT);
• collaboration with volunteer services for transportation, shopping, visits, etc.
