Description
Swiss Pallia Confort is a not-for-profit association that proposes to promote access to the principles of a palliative approach at home,
Swiss Pallia Confort and its caregivers aim to offer patients non-medicalized support and comfort care, for the best possible quality of life,
The team also sets itself the mission of accompanying and relieving family caregivers,
The association gives priority to people whose means do not allow them to afford home support, and may ask others for a contribution,
The territory covered is the canton of Geneva.
This approach closely associates all healthcare professionals already involved with patients, and relies in particular on the doctor/nurse pair, the patient's referents.
In the first phase Swiss Pallia Confort intends to focus on the end of life,
The doctor/nurse pair intervenes in the hours following instability, in patients whose wish is to remain in their place of living (home or EMS).
The Swiss Pallia Confort team accompanies patients for 12 hours a day and more depending on the case, and more specifically at night, with half-day accompaniments possible
The team also offers telephone support to schedule an intervention the following day.
The team takes care of patients discharged from hospital to support the end of life at home and make the most of the time left to live.
This home support will be facilitated by the possible presence of family caregivers
Swiss Pallia Confort contributes to ensuring continuity of care, facilitating the organization of the network, of the ecosystem around the patient,
The association wants to encourage skills development, training and exchanges between all partners,
It also wishes to play a role as a catalyst and facilitator of the political approach to palliative care.
