SPITEX THE SUPPORT offers home care and assistance to the population of Gambarogno, Locarno and surrounding areas.  

It also provides support for home economics and care activities. Support aims to respond comprehensively and comprehensively to the patient's needs, ensuring continuity of care and promptness of intervention in case of urgent need. 

Through collaboration with other associations, Il Sostegno strives to assist and accompany the person with terminal illness and support the family. SPITEX THE SUPPORT intends to extend its scope with activities that will go to meet the growing demands of people in need of assistance.

WHERE WE ARE

The team, which is expanding, consists of various professionals: nurses, chronic wound care specialists, social workers, care assistants, CRS health aides, domestic helpers...

 

PERSONAL

Depending on the interventions needed, different professionals in our Spitex such as the nurse, geriatric caregiver, social health worker, care assistant, and SRC health aide will be involved. THE SUPPORT also collaborates with external consultants and specialized professional figures such as the diabetes nurse, dietician, physical therapist, etc.  

 

Professional update and training

 

SUPPORT promotes ongoing staff training that is based on the collection and analysis of training needs. 

Training aims to foster the improvement of our staff's skills in the three dimensions: knowing, knowing how to do, and knowing how to be. 

The caregiver is entitled to quality care that is based on UPDATED, ADAPTED, EFFECTIVE AND ECONOMIC SCIENTIFIC EVIDENCE. 

Any comments inherent in our work are welcome and considered as useful tools to guide training and improve service.

SPITEX THE SUPPORT was established to respond to the needs and problems of people who are in situations of fragility or need and as support for their families.

For us at The Support, the individual is the center of our thinking and commitment.

We believe that caregiving should be comprehensive and meet both physical and psychological needs.

The goal is to ensure the quality of life and safety of the person and the environment in which he or she lives, while also offering ourselves as a support and help to his or her family members.

We address the user over the age of 16 years, with somatic problems, and need for welfare care and/or services.

The services we offer may be temporary, such as following hospitalization for surgery or acute illness or permanent for chronic, disabling illnesses or age-related needs.

Our care places extreme emphasis on maintaining and restoring the resources that enable the person to maintain or reestablish the degree of autonomy set.

PhILOSOPHY

The applied philosophy of care has as its framework the caring concepts according to Virginia Henderson  and  Dorothea Orem, which emphasize the 14 basic needs of human beings.

This type of nursing involves a thorough initial analysis, to get a baseline picture of what are the bio--psycho--socio--cultural characteristics and highlighting the resources and problems of the individual.

A care plan is then developed in collaboration with the family and referring to the context in which the client is placed.

The specific nursing function is to care for the individual sick person as a whole, so as to encourage his or her active participation in restoring and maintaining autonomy and mobilizing his or her resources (teaching, assisting, substitute care).

In agreement with the patient and family members, the care plan and goals to be achieved are defined, always safeguarding

and supporting the patient's independence and quality of life at home. Personalized care according to individual needs is guaranteed.

 

Services offered to users

CASE IN CHARGE OF THE CASE

Following the referral, A NURSE will make a home visit and perform a thorough assessment of the client's needs (second assessment tool).

Nursing and/or family help activities are carefully surveyed and provided only with the agreement of the person himself/herself and his/her family members.

 

EVALUATION INSTRUMENT

The Service is required to define interventions based on a needs assessment tool, the RAI--HC, which is officially recognized by the Swiss Association of Home Care and the Conference of Directors of Health Care. This tool supports our practitioners in identifying problems, finding appropriate strategies and interventions, and assigning the correct professional to his or her case.

 

BASIC CARE

Our care and assistance services are carried out by staff trained and recognized by the Swiss Red Cross.

This is practical help, which may be needed in cases of physical or mental overload, illness or accident with families, the elderly, the disabled or people living alone.

Basic care prescribed by a doctor is covered by the health insurance fund. The client contributes a 10% deductible.

The interventions are carried out after assessing the real needs of the user, taking into account his or her resources and those of family members or care giver.

 

NURSING CARE 

Nursing care is carried out by personnel who are certified according to the provisions of the LaMal Law.

They contemplate care for the sick and counseling for their relatives.

Health care services prescribed by a physician are covered by the health insurance fund.

Clients contribute a 10% deductible.

Care services provided are limited to 60 hours quarterly.

In complex cases, after a thorough needs assessment and in consultation with the doctor, the necessary documentation for requesting additional hours will be sent to the health insurance company.

 

TIME FOR CHARGE TAKING

The nursing staff, following an initial assessment, will determine the type of service and the time required for taking charge, on the actual needs of the pz, taking into account their resources, those of family members or caregiver.

The set time will be evaluated by his health insurer, which is required to make a decision within 14 days.

Insurance coverage is given if the insurer does not raise objections within 14 days of receiving the announcement.

Any objections are communicated to the insured by the insurer.

Care services provided are limited to 60 hours quarterly; in complex cases after a thorough needs assessment and in consultation with the physician, the necessary documentation for requesting additional hours will be sent to the health insurer. Eventual services or taking time not recognized by the Compulsory Basic Insurance will be billed directly to the user according to the specific fee schedule also available on the website.

Outside the services recognized by its Basic Compulsory Insurance (thus charged to the client) Spitex the Support offers many other services that you will find listed below.

If necessary, by prior arrangement with health management,we are able to give you assistance 7 days a week (paid service, not covered by KVG).

If the client has supplementary insurance, some services can be covered by it, subject to prior announcement. 

 

PROCESS OF CARE 

Implementation of the care plan allows the construction of an individualized framework, where problems and potentials, limitations and resources of the client in fulfilling activities of daily living emerge.

Based on the predetermined goals, a series of specific care actions are implemented.

The treatment plan includes 5 stages:

1. The formulation of current and potential care problems
2. The assessment of current resources
3. The formulation of goals
4. The planning of care actions
5. The evaluation of the effectiveness of interventions and care undertaken

 The documentation of care has its traceability through our nursing record.

New treatment experiences and technologies allow us to improve the prevention and treatment of pressure sores by ensuring the closure of skin lesions in the shortest possible time to current knowledge, minimizing, scarring outcomes.
